Facility Evaluation Report
Licensing Program Analyst (LPA) David Marrufo conducted an unannounced Case Management - Legal/Non-Compliance visit and met with Program Director Dulce Moreno.
The purpose of the visit was to ensure the facility is adhering to the compliance plan submitted to Community Care Licensing (CCL) after a non-compliance meeting held on 12/27/2024.
Based on the non-compliance plan on 12/27/2024, the licensee will develop a plan ensuring that staff are protecting safety, health, morals, welfare of the participants. Participants are accorded their personal rights and free from any form of abuse, which includes staff training on topics such as personal rights, abuse, mandated reporting. Licensee and administrator's plan in operating the facility according to regulations that ensure the facility provides the care and supervision, and meet the client's needs. Licensee to administer training to staff on the protocol in seeking medical attention.
During visit, LPA Marrufo interviewed seven staff and asked them about the training they have received about client abuse and ensuring the rights of residents.
LPA Marrufo reviewed staff in-service training for the following topics and dates: Client Rights, conducted on 12/17/2024; Wheelchair and Vans Tie Downs, conducted on 05/16/2025, Behavioral Support/De-escalation/Safety, conducted on 06/27/2025.
No deficiencies were cited at this time as per California Code of Regulations Title 22. This report was reviewed with Program Director Dulce Moreno and a copy of this report was provided.
