Facility Evaluation Report
Licensing Program Analyst (LPA) Maria (Mita) Partoza conducted an unannounced annual required inspection and met with administrator (ADM) Eleanor Basa and stated the purpose of the visit.
LPA was greeted by 2 staff (S1 and S2) during the visit S1 notified of LPAs arrival. ADM arrived shortly thereafter.
The facility is licensed to serve six (6) adults developmentally challenged (level 7) adults ages 18-59 years, 6 maybe non-ambulatory. LPA observed 1 out of 3 residents present at the facility and 2 out of 3 resident are attending the day program. ADM stated that R1 does not attend regular day program, however, S2 is R1s 1 on 1 in lieu of a day program. S2 arrives at 8:00 am an leaves at 3:00 p.m. S2 stated that R1 is non-verbal.
LPA inspected the facility inside and outside, including common areas, resident rooms, kitchen, bathrooms, driveway, and outdoor areas. Outdoor ramps were sturdy and maintained; free from obstructions and tripping hazards. The laundry appliances were functional, and cleaning supplies were secured. The hallways leading to the resident bedrooms were well lit and free from obstructions. Indoor temperature was within acceptable range of 70°F.
The kitchen surfaces was sanitary and organized, and has a microwave and vent combination. The knives are inaccessible to residents and chemicals stored at the bottom of the sink cabinet were locked. Food supply met requirements for 2 days perishable and 7 days non-perishable. Kitchen water temperature measured at 118.4°F to 119.1°F.
