Facility Evaluation Report

Licensing Program Analyst (LPA) Maria (Mita) Partoza attempted to conduct an unannounced 1 year required inspection visit at the facility.

Upon arrival at 11:15 a.m. LPA knocked on the door and rang the doorbell and waited for someone to come to the door. LPA did not get a response. LPA called the licensee/administrator (LIC/ADM) Eleanor Basa, who confirmed that no staff or resident are in the facility. Staff and residents will be at the facility around 3:00 to 3:30 p.m. LIC/ADM stated that he/she is more or less an hour away from the facility currently is not able to leave the appointment.

LPA will conduct the inspection at a later date/time. LPA left the facility at 12:15 p.m.
