Facility Evaluation Report

Licensing Program Analyst Manuel Monter conducted a POC case management visit to clear deficiencies cited on April 23, 2026 and met with Staff Mae Alaban. LPA explained the purpose of the visit.

The Facility was cited the following Type A and Type B deficiencies on April 23, 2026, during the conclusion of a complaint investigation for complaint ( 26-AS-20260420164411)

80087 Buildings and Grounds (a)(1) - Type A

80065 Personnel Requirements (f) - Tybe B

LPA received plan of corrections for type A and Type B deficiencies on April 27, 2026. Deficiencies cleared during todays visit. POC cleared letter provided to ADM.

LPA spoke to Administrator Remedios Zacarias via phone call. ADM stated the home has already been visited by the pest control company. ADM stated the pest control company is scheduled to visit again in 2 weeks to spray as well. ADM stated she would send full copy of the pest control bill/contract. ADM stated she and her facility staff will inspect the mattress and adjacent furniture for signs of bed bugs every morning for the next 2 weeks, tracking the fact that they are checking an cleaning said areas in the resident bedrooms. (LPA requested a copy of this log by sent every week.) ADM stated if she see's signs of bed bugs, she will contact the pest control company again.

No deficiency was cited during todays visit. This report was reviewed with Administrator Remedios Zacarias. A copy of this report was provided.
