The inspector’s account

Licensing Program Analyst (LPA) Maria (Mita) Partoza conducted an unannounced annual required inspection and met with administrator (ADM) Shu Jen Collado and stated the purpose of the visit.

The facility is licensed to serve six (6) adults developmentally challenged adults ages 18-59 years ambulatory only. During today's visit 07/30/2026, LPA observed 1 out of 6 resident was resting 4 out of 6 were not at the facility per S1 the residents were attending the day program and 1 out of 6 left with a staff to go fishing.

LPA inspected the facility inside and outside, including common areas, resident rooms, kitchen, bathrooms, driveway, and outdoor spaces and storage areas. Outdoor areas were free of hazards; laundry appliances were functional, and cleaning supplies were secured and hallways were clear and well-lit .Indoor temperature was within acceptable range of 74°F.

The kitchen was sanitary and organized; knives were locked and not accessible to residents without supervision. Food supply met requirements (2 days perishable, 7 days non-perishable). Kitchen water temperature measured at 119.8°F. Medications are locked and inaccessible to residents. Facility has first aid kit that is properly stored and complete.

LPA inspected resident rooms and observed rooms to be organized, and sanitary and had adequate storage for resident's personal belongings. Bathrooms had bars and non-skid mats. Bathroom water temperature measured at 119.8°F.
