Facility Evaluation Report

Licensing Program Analyst (LPA) Steve Chang conducted an unannounced annual inspection, and met with Administrator (ADM) Gleen Mesa.

LPA observed 2 staff and 1 resident in the facility. LPA reviewed 2 resident files and 2 staff files. LPA observed licensee, Administrator Certificate, and personal rights posters at the main entrance.

LPA toured the facility with ADM inside and out. LPA inspected living room, family room, laundry area, dinning area, kitchen. There are 3 restrooms, 2 staff live-in rooms, and 4 resident rooms in the facility. Two days perishable foods and seven day nonperishable foods were observed sufficient. Room temperature was observed at 71 degree F, hot water temperature was observed at 108 degree F. The temperature of the freezer was observed at 0 degree F and the temperature of the refrigerator was observed at 41 degree F. Medication cabinet, knives closet and cleaning products for washing dish were observed locked. The facility was equipped with fire alarm, smoke and carbon monoxide detectors. Carbon monoxide detector was tested, and was working. Fire extinguisher was serviced on 7/29/2025.

First aid box, flash lights, and night nights were observed in the facility. The last time the facility conducted the fire drill was on 10/18/2025. Front yard and backyard were inspected. There was no obstruction to block the walkways. One storage room was observed at backyard.

No citation noted for today's visit. Exit interview was conducted with ADM. This report was provided to ADM for signature. A copy of this report was provided to ADM.
