Licensing Program Analyst Manuel Monter conducted an unannounced Case management – incident visit, regarding an incident report that was sent regarding resident R1. LPA met with Administrator Ann Marie R Zarraga and explained the purpose of the visit.
On June 2, 2025, the Department received an incident report, regarding the resident, John Alaoen, referred to as R1. The incident report stated the following:
5/30/2025 5:00PM- Witness W1 came over to visit all the residents of Dignity Care Home. During visit, one of the residents was having his/her behaviors. Resident 1 was crying and was very agitated…The staff and the Personal assistant (PA) of R1 … led the other residents to their rooms. But they kept coming out.
This made Resident 1 more agitated …R1 saw a wooden spatula on the kitchen table and kept picking it up and hiding it under the couch. The PA would take it away from him/her and put it on the table. When R1 was being redirected to go to his/her room, R1 resisted and wanted to stay in the living room. The PA then took the spatula and pretended to threaten and hit R1. PA banged the spatula on the table and on the wall to make R1 obey to go to his/her room. The staff approached the PA and reminded him/her that threatening R1 is not an acceptable way of managing John’s behavior. “
