Facility Evaluation Report

On 07/29/2026, San Bruno Regional Office-San Jose Unit conducted a non-compliance conference meeting with Licensee Mustafa Sabankaya and Administrator (ADM) Prunella Cardozo, and Administrator (ADM) Soujanya Rao. Present in the meeting were Regional Manager (RM) Jackie Jin, Licensing Program Manager (LPM) Christine Kabariti, and Licensing Program Analyst (LPA) Marcela Yanez.

During the non-compliance meeting, the following serious violations were discussed: 80087(g)-Buildings and Grounds, 80088(e)1) Furniture, Fixtures, Equipment, and Supplies, 80065(f)(4), 80065(c)-Personnel Requirements, 80075(b), 80075(f) Health Related Services, 80072(a)(2) Personal Rights, , 80061(b)(1)(D)- Reporting Requirements, 80087(a)-Building and Grounds.

The following document was requested to submit to the Department: LIC309 - Administrative Organization and exception request documents for resident (R1) by August 5, 2026.

During this meeting, the compliance plan was developed and discussed with the licensee which includes

more frequent monitoring inspection visits to ensure compliance with this compliance plan and Title 22
