Facility Evaluation Report
Licensing Program Analyst (LPA) Marcela Yanez conducted an unannounced Required 1 Year visit and met with Prunella Cardozo, Administrator (ADM). LPA announced the purpose of the visit. LPA observed 4 residents in dining area and 4 residents in front patio.
During visit, LPA toured the facility inside and out. LPA observed the kitchen and dining area. LPA observed a housekeeping closet for cleaning supplies. LPA toured the locked medication room.
ADM stated the facility does not prepare food on site it is prepared at Ali Baba #1 #430700017. The facility does not store any knives since food is not prepared here. LPA observed perishable food supply of at least two days and a non-perishable food supply of at least seven days. LPA observed residents personal tubs with snacks in the kitchen that staff will get for them.
LPA and ADM toured 7 random resident bedrooms #1, #12, #14, #17, #26, #20, #23. Each bedroom had available bedding and clothing storage areas as well as functioning lights.
LPA observed R1 bedroom #20 to have a bottle of hyrdrogen peroxide and R2 in bedroom #5 had a bottle of toilet cleaner. ADM removed items a were placed in locked room. ADM stated both residents purchased items without notifying Facility staff. LPA toured two out of two resident bathrooms. The water temperatures in the bathroom sinks measured with thermometer at 142.8 degrees F at 9:00 AM and measured 131.0 degrees F at 11:05 AM.
