Facility Evaluation Report
Licensing Program Analyst Marcela Yanez conducted an unannounced case management visit. LPA announced the purpose of the visit. LPA met with Frank Ebo, Designated Administrator. LPA observed 3 staff and 3 residents in the lobby.
On 11/17/25 LPA Yanez conducted a case management visit regarding a incident that occurred on 11/08/25 where R1 stated he/she was assaulted.
On 12/01/25 LPA Yanez watched video footage of hallway where residents room is located. LPA did not observe anyone entering residents room prior to resident stating he/she had been assaulted. DA stated R1 had another incident of hallucinations on 11/28/25. DA stated that the facility is in communication with the R1s Case Manager and is aware of incidents.
LPA determined the above incident does not require further investigation
No deficiencies were cited per California Code of Regulations, Title 22.
