Facility Evaluation Report
Licensing Program Analyst (LPA) Marcela Yanez conducted an unannounced Case Management Visit. LPA met with designated Administrator (DS) Frank Ebo on behalf of Administrator (ADM) Prunella Cardozo who was contacted by phone. LPA announced the purpose of the visit.
On 11/10/25 the department received an incident report for Resident (R1) that occurred on 10/08/25 who stated he/she had been sexually assaulted. ADM stated that the resident had a history of stating he/she was sexually assaulted. ADM stated R1s case manager had informed ADM prior to moving into the facility. ADM stated that the facility staff reviewed the cameras in the hallway and observed nobody entering residents room when he/she stated they were assaulted. LPA requested copy of video footage of hallway leading into R1s room.
During visit LPA interviewed ADM over the phone, Staff S1 and 2 residents. .
LPA determined the incident requires further investigation.
No deficiencies were cited today. This report was reviewed with Frank Ebo, Designated administrator and a copy of the report was provided.
