Facility Evaluation Report
Licensing Program Analyst (LPA) Manuel Monter conducted an unannounced annual inspection visit, and met with Program Director (PD) Maria Canizales . During the visit, PD stated the facility has 8 staff, and 32 residents.
LPA toured the facility inside out with PD which included the Living room, kitchen, dining room, resident restrooms and residents bedrooms. The staff area of the facility was also inspected. The front yard and backyard were inspected. There was no obstruction to block the walkways.
While touring the facility, LPA observed bedrooms 14,11,10, and 4 had blinds for the windows that are damaged. PD stated she is aware and has it scheduled with maintenance to have them replaced and the replacement blind have already been ordered. While touring resident bedroom 1, LPA observed a green container of "Gain" laundry detergent (Photograph was taken). PD stated resident R3 does his/her own laundry and refuses to use the laundry detergent the facility provides.
Two-day perishable food supplies and seven day nonperishable food supplies were observed. LPA observed the medication storage area, knives storage area, and cleaning product storage area as locked and inaccessible to residents in care. Room temperature was at 70 degrees F, and hot water temperature was measured at 118 degrees F in resident bathrooms.
Fire extinguisher was serviced in March 7, 2024. The facility was equipped with smoke and carbon monoxide detectors. PD stated the centralized sprinkler system was last inspected on March 11, 2024. LPA observed facility first aid kit and facility fire/earthquake drill log. The facility's last drill was on November 8, 2023.
