Licensing Program Analyst (LPA) Christine Dolores arrived at the facility unannounced and met with Administrator, Renee Sabankaya.
The purpose of the visit was to deliver an immediate exclusion letter to exclude an employee at the facility.
The letter was handed to the Administrator during today's visit.
This report was reviewed with Administrator, Renee Sabankaya and a copy of the report provided.
