Staff do not meet the qualifications to care and supervise residents in care.

Licensing Program Analyst (LPA) Christine Dolores arrived unannounced to open the initial complaint investigation. LPA met with Program Director, Maria Canizales. The following documents were obtained for this complaint investigation to include the client roster, staff roster, staff schedule from 07/10/23 – 07/16/23, staff (S1 – S6) application for employment, and training records.

During visit, LPA interviewed staff (S1) and reviewed staff (S1 – S6)’s personnel files and training records. Based on record review and interview, staff (S1 – S6) provides direct care and supervision to clients and does not obtain an active first aid certificate. The facility is currently working on scheduling staff for first aid and CPR certification.

The Department has investigated the above allegation and the preponderance of evidence standard has been met. Therefore, the Department found the above allegation to be SUBSTANTIATED. A deficiency was cited per California Code of Regulations, Title 22. See LIC9099-D. This report was reviewed with Program Director, Maria Canizales and a copy of the report and appeal rights were provided.
