Facility Evaluation Report
Licensing Program Analyst (LPA) Steve Chang conducted an unannounced annual inspection visit, and met with Administrator (ADM) Tina Cristobal. The facility is a level 7 facility. All residents were out for day program, one staff was observed in the facility. LPA reviewed 2 resident files and 2 staff files.
LPA toured the facility inside and out with ADM. License, Administrator Certificate, and personal rights posters were observed in the facility.
Family room, living room, kitchen, dining area, 2 restrooms, 1 staff live-in room, 3 resident rooms, laundry room and garage were inspected. Two day perishable food supplies and seven day nonperishable food supplies were observed sufficient. The temperature of the freezer was observed at 0 degree F, and the temperature of the refrigerator was observed at 40 degree F. Medication closet, knives closet, and chemical closet were observed locked. Room temperature was at 73 degree F, and hot water temperature was at 119 degree F in facility. Fire extinguisher was serviced on 8/5/2026. The facility was equipped with fire alarm system, smoke and carbon monoxide detectors. Carbon monoxide detectors were tested by ADM, and were working. First aid box, night lights, and flash lights were observed in the facility. The facility has emergency light system. The facility 2 restrooms have motion detecting light system.
Front yard and backyard were inspected. There was no obstruction to block the walkways. One storage room and two large storage cabinets were observed locked in the back yard. The facility had earthquake drill on 7/16/2026 and fire drill on 7/25/2026.
