Facility Evaluation Report

Licensing Program Analyst (LPA) Manuel Monter conducted an unannounced case management - Incident visit regarding a self-reported Incident Report for an incident which occurred on 1/29/2025. LPA also followed up regarding the 3 day eviction for resident R3 and R3's current status. LPA met with Lead Staff (LS), Shierre Medula and stated the purpose of the visit.

On 1/30/2025, the Department received an Incident Report stating on 1/29/2025 regarding alleged physical abuse by staff (S1) towards resident (R1).

LPA Monter interviewed 1 staff and 3 residents.

During today's visit, LPA observed resident R3 at the facility. LS stated R3 is still currently living in the home. LS stated R3's family member is currently looking to move R3. LS stated R3's service coordinator is aware.

The report was reviewed with Lead Staff, Shierre Medula and a copy of the report was provided.
