Facility Evaluation Report
Licensing Program Analyst (LPA) Marcela Yanez conducted an unannounced Required 1 Year visit and met with Jean Berris, Administrator. LPA announced the purpose of the visit. LPA observed 2 staff . 4 out of 4 residents at day program.
During visit, LPA toured the facility inside and out. LPA toured the garage area and observed food storage areas and locked cabinets for cleaning supplies. LPA observed the kitchen area and observed locked cabinets sharp objects, and cleaning supplies. LPA observed locked cabinet in living room for medications. LPA observed perishable food supply of at least two days and a non-perishable food supply of at least seven days.
LPA toured three resident bedrooms. Each bedroom had available bedding and clothing storage areas as well as functioning lights. LPA observed 1 staff room. ADM tested the smoke detector in the hallway and found the smoke detector to function properly when tested. LPA toured 1 resident bathroom that had available soap and paper towels and functioning lights. The water temperature in the bathroom sink measured with thermometer at 114.4 F.
During tour LPA observed the back patio to be converted into a room with windows and 2 exiting doors. ADM stated the construction was just completed in 12/2025. Licensee was contacted by phone who stated she has a permit for construction but did not notify licensing regarding any alterations to the facility. ADM stated she will submit a new facility sketch for a new fire clearance and provide permits.
