Facility Evaluation Report
Licensing Program Analyst (LPA) Marcella Tarin conducted an unannounced annual inspection and met with Administrator (ADM) Johann Deguzman. LPA stated the purpose of the visit. LPA observed 2 staff and 1 client in the facility and 4 clients were at day program.
LPA toured the interior and exterior with the ADM to include but not limited to the living room, dining room, garage, and client rooms. All exits and passageways were free and clear of obstructions. LPA observed 2 days of perishable foods and seven day non perishable foods. LPA observed medications, sharps and toxics were locked and inaccessible to clients in care. The facility fire extinguisher was serviced on 11/18/2025. The facility was equipped with a fire alarm system and carbon monoxide detectors. Fire alarm and carbon monoxide detectors functioned properly when tested by the ADM. The facility emergency drill log was reviewed. The facility conducted an emergency drill on 12/30/2025.
LPA toured 3 client rooms. LPA observed 1 staff live-in room. LPA measured the water temperature in 2 client bathrooms with a range from 113.3 F to 117.1 F.
LPA reviewed 2 client records, 2 client's P & I, and 2 clients Centrally Stored Medication and Destruction Records (CSMDRs).
No deficiencies were cited during today's visit per California Code of Regulations, Title 22. An exit interview was conducted with ADM and copy of this report was provided.
