Facility Evaluation Report
On 4/8/2026, LPA Grace Donato conducted an unannounced Required 1 Year visit. LPA met with Administrator, Roger Caday. LPA explained the purpose of the visit. .
During visit, LPA toured the facility inside and out. LPA observed the room temp was 70 degrees F. LPA toured the garage area and observed food storage areas and locked cabinets for cleaning supplies. LPA observed the kitchen area. Locked cabinets for medications and sharp objects. LPA observed perishable food supply of at least two days and a non-perishable food supply of at least seven days.
LPA toured three resident bedrooms.Residents bedrooms were observed to be well organized and fully furnished with adequate lighting. The water temperature in the bathroom sink measured with thermometer at 108 F. Room temperature is at 70 deg F.
Two resident records and two staff records were reviewed. All medication logs are complete and updated. Facility has an updated log for emergency drill done quarterly..
No deficiencies cited today. Report is reviewed and copy is provided.
