Facility Evaluation Report
Licensing Program Analyst (LPA )Steve Chang conducted an unannounced case management visit and met with assistant Administrator (AADM) Roger Caday and House Manger (HM) Olivia Lazo.
The purpose of today's visit is to check and review the improvement/enhancement of the facility.
LPA observed a new ramp to enter the facility. LPA observed new replaced main entrance screen gate, new replaced main entrance door, a new replaced resident room door and a new replaced door to laundry room. LPA observed new replaced networked fire alarm detectors and carbon monoxide detector. LPA observed a new secured guarded gate for the heater. LPA observed 3 new plug outlets in kitchen to avoid to use extention cords.
LPA reveiwed existing city permit with AADM.
AADM stated the facility tries the best to ensure saftey and health for the residents and staff.
