Facility Evaluation Report

Licensing Program Analyst (LPA) David Marrufo conducted an unannounced Case Management Visit and met with Sabrina Setz, Assisted Living Director.

The purpose of the visit was to follow up on a deficiency cited on 03/14/2025 as part of a complaint received by the department on 03/03/2025. The facility was cited for having personal resident records unsecured in an unlocked office room.

The facility completed a Plan of Correction on 03/27/2025 which included ensuring that the financial department office will be locked at all times when it is unoccupied by a staff.

During visit on 07/23/2025, LPA observed that the door to the financial department office was locked. A staff unlocked the financial department office door and LPA Marrufo observed the office. The staff stated that the office door is always kept locked when unoccupied.

No deficiencies were cited at this time as per California Code of Regulations Title 22.
