Facility Evaluation Report

Licensing Program Analyst (LPA) Christine Dolores arrived unannounced to conduct a case management - other visit. LPA met with Associate Executive Director, Ash Sharma.

The purpose of the visit is to deliver an immediate exclusion letter for staff (S1). LPA explained the purpose of the letter. AED confirmed S1 is not currently working in the facility and understands that S1 can no longer work in licensed facilities.

The immediate exclusion letter was handed to AED.

No deficiencies were cited today per California Code of Regulations, Title 22. This report was reviewed with Associate Executive Director Ash Sharma and a copy of the report was provided.
