Licensing Program Analyst (LPA) Steve Chang conducted an unannounced case management visit and met with Administrator (ADM) Donna Lagman.
LPA toured the facility inside out with ADM. ADM stated all residents were relocated. No resident was observed in the facility. The facility is in non operational status.
No construction was observed in the facility. ADM stated the facility will have new interior renovation soon.
ADM stated the facility plans to reopen on 8/1/2026.
Exit interview was conducted with ADM. The report was provided to ADM for review and signature. A copy of the report was provided to ADM.
