Facility Evaluation Report

Licensing Program Analyst (LPA) Simi Rai conducted an unannounced Case Management - Incident visit in regards to an Incident Report the Department received on 7/28/2025. LPA Rai met with Care Staff, Jose Roberto Barba and stated the purpose of the visit. LPA Rai spoke with Administrator Sashi Kumar and received verbal confirmation for Care Staff, Jose Roberto Barba to sign today's report.

On 7/28/2025, the Department received an Incident Report stating on 7/25/2025 at approximately 5:40pm, staff S1 slapped resident R1 twice on the right side of the face, leaving a visible red mark. This incident was observed by staff S2 and S3 and they intervened and applied an ice pack to R1's face. Per report, S1's employment was terminated and no longer works at the facility.

During today's visit, LPA Rai conducted interviews with 1 staff (S2) and 6 residents (R1-R6).

LPA Rai reviewed staff (S1)'s facility records and training.

At this time, LPA Rai determined this case management needs further investigation.
