Facility Evaluation Report
Licensing Program Analyst (LPA) Simi Rai conducted an unannounced Case Management - Incident visit in regards to an Incident Report the Department received on 7/7/2025. LPA met with Administrator, Sashi Lata Kumar and stated the purpose of the visit.
On 7/7/2025, the Department received an LIC 624 Incident Report stating there was alleged physical abuse on 7/3/2025 wherein resident (R1) reported to facility staff that staff (S1) had pinched the resident's chest, near the left nipple. Facility staff did not observe an open wound.
During today's visit, LPA Rai conducted interviews with 2 staff and 6 residents.
At this time, LPA Rai determined this case management needs further investigation.
This report was reviewed with Administrator Sashi Lata Kumar and a copy of the report was provided.
