Facility Evaluation Report

Licensing Program Analyst (LPA) Simi Rai conducted an unannounced visit to amend a report and clear the deficiencies issued on 07/16/2026. LPA Rai met with the Administrator Designee, Joey Barba and stated the purpose of today’s visit.

During today's visit, LPA Rai cleared one deficiency issued during today's visit. During the investigation, Licensee provided a written plan of action to ensure staff were present at the facility to provide care and supervision.

During today's visit, LPA Rai amended the report dated on 07/16/2026.

LPA Rai provided Licensee Letter of Deficiency Citations Cleared for deficiencies issued during today's visit.

Exit interview conducted with Administrator Designee, Joey Barba and a copy of the report was provided.
