Facility Evaluation Report
Licensing Program Analyst (LPA) Simi Rai conducted an case management visit to follow up on issues of the building structure of the facility. LPA Rai met with Licensee/Administrator, Xiuyan Chen and stated the purpose of today's visit. LPA Rai observed 5 staff members at the facility and they have obtained Criminal Background Clearance and were associated to the facility. LPA Rai observed 5 residents at the facility.
During today's visit, LPA Rai observed the three storage areas of the facility. Storage #1 is located in the garage which is converted into a bedroom and bathroom. Mrs. Chen stated the staff use storage #1 as a rest area. LPA Rai observed a sofa with a mat located on the corner of the room. LPA Rai observed
Storage #2 is located in the backyard as a stand-alone building with the roof attached to the facility building. Mrs. Chen stated storage #2 has extra food for the residents. LPA Rai observed a working fridge with refrigerator and freeze with food items placed inside. Storage #3 is located on the side of the home and has been converted to a bedroom and attached bathroom. LPA Rai observed storage items placed inside, such as extra wheelchair and household items.
Based on the Fire Clearance dated 4/3/2023, which states the garage (Storage #1) is not approved for use. The Fire Clearance does not state for Storage #2 and Storage #3 to be approved for use at this time.
At this time, the three storage areas of the facility do not have building permits for the space to be used and should be off-limits. Mrs. Chen will submit a plan of action address how three locations should be locked and inaccessible to staff and residents. The plan will be submitted to Community Care Licensing by Tuesday, May 5th 2026 via email to LPA Rai.
