Facility Evaluation Report

Licensing Program Analyst (LPA) conducted an unannounced visit at the facility and met with Administrator (ADM) Bhupinder Kaur.

Upon LPA's arrival, LPA observed a staff (S1) who is associated with the facility. LPA observed 3 residents (R1 - R3) in the facility and 1 residents (R4) went to day program.

During LPA spoke with S1. R1 was picked up by a case manager for doctor appointment. R2 was picked up by conservator and family member for doctor appointment. R3 was sleeping at the bedroom.

LPA interviewed ADM. ADM stated there are 2 staff including 1 live-in staff . ADM stated he/she has ARF certificate and currently manages the facility. ADM stated he/she works at the facility as needed. ADM stated the facility already passed the fire clearance.

LPA obtained a copy of the fire clearance report and a copy his/here valid ARF Certificate.

ADM stated he/she already associated with the facility ADM stated he/she has acquired the facility and the property of the house. ADM stated he/she applied for new license in October 2025.

ADM stated the facility is well managed and operates normally.

No citation noted today. Exit interview was conducted with ADM. The report was provided to ADM for review. A copy of the report was provided to ADM.
