Facility Evaluation Report
Licensing Program Analyst (LPA) Simi Rai conducted an unannounced case management visit to follow up on exception requests for clients for the use of postural support. LPA Rai met with Administrator Designee, Marta Malapitan and stated the purpose of the visit.
During today's visit, LPA Rai reviewed facility file records for 39 clients who require an exception request. 30 Out of 39 clients have non-ambulatory status. Based on review of facility's current Fire Clearance Approved 09/28/2021, the facility can have 25 non-ambulatory clients at the facility.
During today's visit, Administrator Designee, Marta stated the facility does not have an exceptions on file for the clients and they are in the process of obtaining the necessary information to submit exception requests to the Department.
Administrator Designee, Marta stated to submit an Plan of Correction (POC) by POC due date which is tomorrow 04/28/2026 by 11:59pm. Administrator Designee, Marta stated the POC will address how the facility will be in compliance with current Fire Clearance the facility has received of having 25 non-ambulatory clients in the facility.
Failure to correct the cited deficiency(ies), on or before the Plan of Correction (POC) due date, may result in a civil penalty assessment.
