Facility Evaluation Report

Licensing Program Analyst (LPA) Simi Rai conducted an unannounced case management visit to follow up on exception requests for clients for the use of postural support. LPA Rai met with Administrator, Patrick Laurel and Administrator Designee, Marta Malapitan and stated the purpose of the visit.

During today's visit, LPA Rai toured 4 activity rooms and observed 21 residents who required wheelchair seat belt or helmet for safety.

During today's visit, Administrator Designee, Marta stated the facility does not have an exceptions on file for the clients and they are in the process of obtaining the necessary information to submit exception requests to the Department.
