Facility Evaluation Report

Licensing Program Analyst (LPA) Steve Chang conducted an unannounced visit and met with Administrator (ADM) Patrick Laurel.

The purpose of today's visit is to check the new additional louder door alarm for each exit door in the facility.

LPA checked the 4 exit doors with Program Manager (PM). The facility added additional louder door alarm for each exit door. LPA checked with PM and they were working.

LPA interviewed ADM, PM, Office manger, 2 direct care staff, and 1 administration assistant.

ADM confirmed he/she owns this facility and another two care homes.

No citation noted today. Exit interview was conducted with ADM. The report was provided to ADM for review and signature. A copy of the report was provided to ADM.
