The inspector’s account

Licensing Program Analyst (LPA) Simi Rai conducted an unannounced Case Management visit to conduct a wellness check at the facility. LPA Rai met with Administrator Designee, Marta Malapitan and Office Manager, Aquinas Malapitan and stated the purpose of today's visit. LPA Rai spoke with Licensee/Administrator over the phone and stated the purpose of today's visit. LPA Rai observed 12 staff and 0 clients at the facility.

On 05/26/2026, the Department was informed by the Fire Department regarding the new Fire Clearance request which was submitted on 05/01/2026 was denied due to building permits of the facility.

On 05/26/2026, Licensee/Administrator Patrick Laurel submitted a plan of action to the Department and San Andreas Regional Center (SARC) regarding temporarily closing operations of the facility until building permits were resubmitted to the city and other necessary agencies. The plan of action included a copy of the letter sent to clients and their authorized representatives regarding the temporary closure of the facility and temporary remote program activities conducted through out the day in lieu of in person programming.

During today's visit, LPA Rai observed the front gates closed with a sign which read "Temporarily Closed". LPA Rai conducted a tour of the inside and outside of the facility and observed 0 clients at the facility and 12 staff present inside which were either conducting remote services through Zoom or completing their staff training. LPA Rai also reviewed the facility's "Temporary Remote Program Activities" which will start at 8am and end at 2pm and included activities such as "Social Skills", " Creative Activity", and "Community Integration".

No deficiencies were cited per California Code of Regulations, Title 22. This report was reviewed with Licensee/Administrator, Patrick Laurel and a c opy of the report was provided.
