Facility Evaluation Report
On 4/13/2026, LPA Grace Donato conducted an unannounced annual visit and met with Administrator Paul Aliason. LPA explained the purpose of the visit.
Facility is a single story house with 4 resident bedrooms. LPA toured the facility inside and outside including all of resident rooms, living room and kitchen area. Residents bedrooms were observed to be well organized and fully furnished with adequate lighting. Resident bathrooms were observed to be in good repair. Sharps and toxic materials were observed locked and inaccessible to residents. Food supply in was observed with an adequate two day perishable and seven day non-perishable food supply. Carbon monoxide monitor is working and fire extinguisher, which was last serviced on 10/10/2025, were present at the facility.
LPA reviewed three resident files and three staff files and all are complete and updated. Medication review was done, and all medications are accounted for, and centrally stored medication records are updated. Emergency drills are logged and done quarterly. PNI money was counted and complete.
LPA requested the following to be emailed LIC308, LIC500, updated Facility floor plan & LIC610.
No deficiencies cited today. Report is reviewed and copy is provided.
