This is an official report of an unannounced visit/investigation of a complaint received in our office on

03/07/2025 and conducted by Evaluator Jaime Vado

COMPLAINT CONTROL NUMBER: 26-AS-20250307123002

- Staff allowed client to engage in self-harm behavior resulting in hospitalization

On 05/20/2025, Licensing Program Analyst (LPA) Jaime Vado conducted an unannounced complaint investigation visit in order to deliver findings regarding the allegations received. LPA met with Administrator - Prunella Cardozo and explained the purpose of today's visit.

During the investigaiton documents are reviewed, interviews are conducted, and observations are made. It cannot be determined if the allegations took place as described. Based on logs of interactions and interviews with staff, it cannot be determined if the facility dismissed the resident's ideations. Medications were given per logs reviewed and staff interactions to aid the resident. This allegation is unsubstantiated.

Based on these observations, the above allegations are UNSUBSTANTIATED.

Although the allegations may have happened or is valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the above allegations are unsubstantiated at this time.
