The inspector’s account

Licensing Program Analyst Marcela Yanez conducted an unannounced case management visit. LPA announced the purpose of the visit. LPA met with Frank Ebo, Designated Administrator. LPA observed 3 staff and 3 residents in the lobby.

On 11/19/25 the department received an incident report for Resident (R1) who had obtained a kitchen butter knife used during mealtime. DA stated that R1 had threatened another Resident (R2) who frequently goes to other residents rooms and asks for cigarettes. DA stated that the staff asked him/her to look for the knife when doing rounds. DA saw the resident and approached R1 and asked for the knife back. DA stated R1 refused and stated that he/she would use knife if DA got closer. DA then called the police and police arrived and retrieved the knife and arrested R1. R1 did not return to facility.

During visit LPA was shown a replicate of the knife that was involved in incident. LPA determined the above incident does not require any further investigation. The knife is part of the utensils provided to residents to use during meal time.

No deficiencies were cited per California Code of Regulations, Title 22.
