Facility Evaluation Report
Licensing Program Analyst (LPA) Marcela Yanez conducted an unannounced case management visit. LPA announced the purpose of the visit and met with Prunella Cardozo, Administrator (ADM).
On 12/08/25 the department received a incident report involving resident on residents altercation that occurred on 12/05/25.
On 12/05/25 two residents were arguing in front of staff and were insulting one another. Staff immediately observed residents to ensure incident did not escalate. Resident (R1) left the facility first and Resident (R2) followed, staff followed behind both of them. The staff reached clients in the street at the corner from the facility. Upon arrival staff found 1 client had an injury on his/her hand from R1. R1 returned to his/her room at facility and informed S3 he/she stabbed someone and hid weapon. S1 and S2 stayed with R2 until emergency services arrived. R2 was not seriously injured and suffered a minor cut on hand.
ADM stated the weapon was determined to be a box cutter. ADM stated the facility does not use box cutters. ADM stated R2 had a pass the night before the incident and might obtained box cutter while on pass. R2 was arrested and taken into custody. ADM stated she began eviction process and resident did not return to facility. The facility is taking precautions by increasing the night shift room checks from 3 to 4.
