Facility Evaluation Report
On 07/24/25 Licensing Program (LPA) Analyst Marcela Yanez conducted a case management visit and met with Prunella Cardozo, Administrator (ADM). LPA announced the purpose of the visit.
On 07/18/25 the department received a incident report that occurred on 07/17/25.
During visit LPA obtained copies of 3 Resident Records including but not limited to Physicians report appraisal needs and services, resident roster, identification and emergency contact information and staff schedule, and staff training.
LPAs determined that the above incident requires further investigation. No deficiencies were cited at this time as per California Code of Regulations Title 22.
This report was reviewed with Administrator, Prunella Cardozo and a copy of the report was provided.
