Facility Evaluation Report
On 07/15/25 Licensing Program Analyst (LPA) Marcela Yanez conducted an unannounced Case Management incident visit. LPA announced the purpose of the visit and met with Prunella Cardozo, Administrator.
On 07/14/25 the department received 2 incident reports that occurred on 07/06/25 and 07/10/25.
During visit LPA obtained copies of 6 Resident Records including but not limited to Physicians report appraisal needs and services, resident roster, identification and emergency contact information and staff schedule.
LPAs determined that the above incidents requires further investigation. No deficiencies were cited at this time as per California Code of Regulations Title 22.
This report was reviewed with Administrator, Prunella Cardozo and a copy of the report was provided.
