- Facility staff neglected resident's physical care
On 01/28/2025, Licensing Program Analyst (LPA) Jaime Vado conducted an unannounced complaint investigation visit in order to deliver findings regarding the allegation received. LPA met with case manager Susana Tovar and explained the purpose of today's visit.
During the investigation interviews were conducted, files were reveiwed, and observations were made. Per interviews conducted and observations were made, the faciilty did attempt to manage the resident's physical care. Interviews showed that staff did encourage the resident to bathe, change clothes, and self care. The resident refused the suggestions. Resident would also some in his/her room which is against house rules but staff also encouraged to not do so but he/she did. Based on information received these allegations are unsubstantiated.
Based on these observations, the above allegations are UNSUBSTANTIATED.
Although the allegations may have happened or is valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the above allegations are unsubstantiated at this time. Report is reviewed with assistant administrator Prunella Cardozo.
