The inspector’s account

Licensing Program Analyst (LPA) Marcela Yanez conducted an unannounced Case Management visit to conduct a Non-Compliance Plan Quarterly Visit and met with Abdullah Kaykha, Administrator (ADM). LPA announced the purpose of the visit. LPA observed 2 staff including ADM and 3 Residents.

The purpose of the visit is to ensure the facility is adhering to the Compliance Plan submitted to Community Care Licensing (CCL) after an informal meeting held on 12/23/24.

Based on the non-compliance plan, Licensee/administrator shall develop a plan which was submitted on 1/22/2025. A revision was submitted on 2/15/2025.

During tour LPA toured the Facility and 3 resident bedrooms which had sufficient lighting, bedding and closet and dresser for residents belongings. LPA observed exits and entrances free and clear of obstruction. LPA observed knives and toxics in locked cabinet. LPA observed 2 days of perishable food supply and 7 day non perishable food supply.

LPA reviewed and received a copy of weekly temperature log last check was on 07/11/25 which stated the kitchen water measured with thermometer at 116.3 and on 07/11/25 bathroom water measured with thermometer at 113.7
