Facility Evaluation Report
On 07/14/25 Licensing Program Analyst LPA Marcela Yanez conducted an unannounced case management visit. LPA announced the purpose of the visit and met with Abdullah Kaykha, Administrator. LPA observed 2 residents and 1 staff including Administrator, Abdullah Kaykha.
On 06/10/25 the department received an incident report for Resident (R1) who stated he/she was sexually abused.
On 06/19/25 LPA conducted an unannounced Case Management visit and obtained documentation and interviewed 2 staff and R1 who stated he/she locks their door while he/she sleeps. Documents reviewed: physicians report, appraisal needs and services, progress notes.
Based on interview and record review LPA determined the case management is considered to be closed and does not require further investigation.
No deficiencies were cited at this time as per California Code of Regulations Title 22.
