Facility Evaluation Report

Licensing Program Analyst (LPA) Marcela Yanez arrived unannounced to conduct a Case Management-Incident visit and met Abdullah Kayhka, Administrator (ADM). LPA stated the purpose of the visit. LPA is following up on Incident Report (IR) submitted to the Department.

On 06/10/25 the Department received a IR for 1 resident. The IR stated R1 had stated he was sexually abused in his/her bedroom while sleeping because he/she woke up wet. R1 called local law enforcement and stated he was assaulted. SJPD came to facility and interviewed R1. ADM stated that officer who interviewed R1 did not take any further action. on 06/18/25 APS interviewed R1. R1 physicians report has a diagnosis of Schizophrenic with secondary diagnosis of autism and a history of Psychotic affective disorder dated 08/19/24. Appraisal needs and services states R1 has history of Paranoia.

ADM had previously emailed LPA with documentation pertinent to the case management and LPA obtained copies of IPP and Appraisal needs and services, Physicians report.

LPA determined this case management requires further investigation.

No deficiencies were cited during today's visit. An exit interview was conducted with ADM Abdullah Kayhka and a signed copy of this report was provided.
