Complaint Investigation Report
Regarding the allegation of facility did not report unusual incident to required parties, RP arrived at the facility 15 minutes after the incident, where none of the staff reported the incident that had just occurred.
LPA interviewed administrator and it was mentioned that a special incident report (SIR) wasn't made for this incident because this is R2s baseline behavior, and the facility staff would make an SIR everyday if they had to write one every time R2 said swear words to staff or other residents. Before reporting incidents to Licensing or San Andreas Regional Center (SARC), administrator or staff need to investigate and know the reasons as to why the incident happened to get the story for all parties concerned.
Based on records review, part of the objective/plan for R2 is to be emotionally stable, minimize yelling, cussing, calling names to other people when angry. Control R2s physical aggression.
Therefore, based on the interviews conducted, files reviewed, and information collected, the above allegations are found to be UNSUBSTANTIATED, meaning that although the allegations may have happened or is valid, there is no preponderance of evidence to prove that the alleged violation occurred.
