Facility Evaluation Report
Licensing Program Analysts (LPA) Marcela Yanez conducted an unannounced visit and met with Administrator (ADM) Shirley Richards. LPA announced the purpose of the visit. LPAs observed 7 Clients and 5 Staff. Upon arriving at the facility LPA observed the front door locked and a plastic chair blocking the entrance. Staff S1 moved the chair and unlocked door.
LPA toured the facility inside and out with ADM which included the main activity area and music room, kitchen, dining room, restrooms. The staff area of the facility was also inspected. The outside of the facility was also inspected. There was no obstruction to block the walkways. LPA observed 2 facility vehicles that take Clients to community outings and to and from home. ADM stated vehicles are serviced for scheduled maintenance as needed.
During the visit, LPA observed clients doing activity in the main area. During tour LPA observed a game room with puzzles and board games a giant screen television, storage area, and cleaning product storage area as locked and inaccessible to residents in care.
Room temperature was at 70 degrees F, and hot water temperature was measured with thermometer 109-116 degrees F in restrooms. LPAs observed paper towels and soap and covered trash cans.
