Facility Evaluation Report

Licensing Program Analysts (LPAs) David Marrufo and Santino Fortes conducted an unannounced Required 1 Year visit and met with Administrator Shirley Richards.

During visit, LPAs toured the facility. LPAs toured the kitchen area, office area, activity rooms, hallways, and bathrooms. The facility is not currently under operation and does not have any admitted clients.

During visit, Administrator Richards stated that facility has received vendorization through San Andreas Regional Center.

No deficiencies were cited as per California Code of Regulations Title 22.
