Facility Evaluation Report
Licensing Program Analyst (LPA) Maria (Mita) Partoza conducted an unannounced annual required inspection and met with administrator (ADM) Arielle Teodoro and stated the purpose of the visit.
The facility is licensed to serve six (6) adults developmentally challenged (level 6) adults ages 18-59 years, all 6 maybe non-ambulatory. LPA observed 0 out of 6 residents. ADM stated that all the resident attends day program. LPA observed 2 staff during the visit. ADM stated that the staff are off once residents leave for the day program and no staff are left at the facility.
LPA inspected the facility inside and outside, including common areas, resident rooms, kitchen, bathrooms, driveway, and outdoor areas. Outdoor areas were free of hazards, ramps were sturdy and maintained. The laundry appliances were functional, and cleaning supplies were secured. The hallways leading to the resident bedrooms were well lit and free from obstructions. Indoor temperature was within acceptable range of 70°F.
The kitchen surfaces was sanitary and organized; LPA observed that vent hood above the cook top is rusty and paint was peeling off. ADM stated that hood vent is scheduled to be replaced by 05/01/2026. The knives are inaccessible to residents and chemicals stored at the bottom of the sink cabinet were locked. Food supply met requirements for 2 days perishable and 7 days non-perishable. Kitchen water temperature measured at 106°F.
