Facility Evaluation Report

Licensing Program Analyst (LPA) Steve Chang conducted an unannounced case management visit to deliver an amended investigation report delivered on 06/25/2026 for the complain of control number 26-AS-20260505145411, and met with administrator (ADM), Tram Truong.

During the visit, LPA observed 6 residents and 4 staff in the facility.

LPA addressed to staff S1 that the purpose of today's visit is to amend the investigation report delivered on 06/25/206 to make clarification to avoid confusion.

The amended investigation was delivered to ADM for review and signature.

Exit interview was conducted with ADM. This report was provided to ADM for review and for signature.
