The inspector’s account

Licensing Program Analyst (LPA) Marcella Tarin conducted an unannounced Case Management-Annual Continuation visit for an annual inspection that was started on 5/18/2026. LPA met with Staff S1 Teresita Deausen. S1 called Administrator (ADM) Jose Roberto Barba. S1 stated ADM was on his way to the facility. ADM arrived at approximately 9:30AM. No clients were present at the facility. ADM stated 6 clients were attending day program.

LPA toured the interior and exterior of the facility with S1 and ADM to include the front and back of the facility, kitchen, 5 client rooms, bathrooms, office and 1 staff room. All 5 bedrooms have a bed, functioning lights, and space for personal belongings. LPA observed all exits and passageways were free and clear of obstruction. LPA observed the facility to be clean, safe, sanitary and in good repair.

LPA observed all knives, chemicals, and medications were locked and inaccessible to clients in care.

LPA reviewed 2 clients Centrally Stored Medication and Destruction Records (CSMDRs).

No deficiencies were cited during today's visit per California Code of Regulations, Title 22. An exit interview was conducted with ADM and a copy of this report was provided.
