Facility Evaluation Report

On 5/8/25 Licensing Program Analyst Santino Fortes conducted a case management visit to follow up on a Technical Advisory note that was given on 4/29/25 during the Annual inspection where two tiles were observed to have minor deteriorating grout around the edges. More grout was added to the deteriorating edges to prevent loosening of the tiles.

No deficiencies were cited during today's visit as per California Code of Regulations Title 22. Exit interview was conducted with ADM. This report was reviewed and a copy was provided to ADM for signature.
