Facility Evaluation Report
On 6/17/2026, Licensing Program Analyst(LPA) John Calandra arrived at the facility to conduct a Case Management visit to follow up on a Exemption denial the Department received for R1. LPA Calandra was greeted by Michael Dunn, Interim Administrator and explained the purpose of the visit.
On June 10, the Department received an Exemption denial for S1 and LPA Murial Han followed up with the Licensee regarding the removal of the employee from the facility. A Confirmation of removal was sent stating the date of removal of the employee as 4/24/2024.
Grant Edelston, Senior Director of Regulatory and Licensing, confirmed via phone call that S1 is not an active employee at the facility.
A copy of the facility's LIC 500 personnel report, staff schedule, and print out of the Licensee's guardian account showing removal of S1 from their roster, was received at the facility.
No deficiencies cited during today's visit.
An exit interview was conducted and a copy of the report provided to the facility representative.
